Referral steps

From referral to follow-up

  1. Send the referral and records

    Use the online referral form or fax sheet. Include the resident’s needs and a coordinator’s direct contact.

  2. Review clinical and coverage needs

    Intake coordinates the referral review, available services, and insurance information.

  3. Schedule the visit

    Plan an in-person or telemedicine appointment based on clinical needs, location, and coverage.

  4. Coordinate follow-up

    Keep the resident and authorized contacts involved as appointments or living arrangements change.

Planning a resident’s care

Can RR manage prescriptions for someone living in our program?

Yes. RR can evaluate residents for outpatient addiction and psychiatric medication management. A clinician reviews the resident’s needs, current medications, and treatment history before making a prescribing plan. Psychiatric care is coordinated through Chattanooga.

Can residents attend by telemedicine?

RR offers clinic and telemedicine appointments. Intake will help plan the visit format based on the resident’s location, clinical needs, and coverage. Include the preferred format when referring.

What if a resident has a time-sensitive medication need?

Call intake at 423-498-2000 and explain the medication concern and any upcoming discharge date. The clinical team must review the case before deciding on prescriptions or changes.

How can our team stay involved after the first visit?

Include a coordinator’s direct phone number and the existing authorization for communication. The resident, RR team, and authorized contacts can coordinate records, appointments, and follow-up as the resident’s living situation changes.
